Step by step survey (web form) for website.

We need to develop a survey (or web form) for our customers, the main purpose is to get information from the customer so we can personalize the product he just bought. we want this to be a step by step form, so the customer won’t see a long tail of questions, instead he will be filling out all the info required in the current step. In he doesn’t complete the current step, he won’t be able to continue. It’s important that the customer can have the ability to pause and resume this later on without losing the information. We also need that the customer upload some files, like jpeg, png, pdf files. Our website is built in wordpress but this development can be done in any other platform o language, we can have it working on a subdomain or subfolder. There will be like 20 questions (steps) and also some of them are logical depending on the customer answer. Example if the answer is yes, we will ask him for more information but if the answer is no, then we will continue to the next step.
