Some Customization in a PHP Base Script which I have Purchased from Codecanyon

PHP Programmer with knowledge of MYSQL Required for this Job. (Preferably from India) I have Purchased a PHP Script - [login to view URL] I have Installed it on my server and Its working fine. Now I want some Customization in this script. I have mentioned Customization requirements in the Given URL. - [login to view URL] or also mentioned below. I will Provide you the original Purchased Script. Kindly Let me know Who can do this job and most IMP thing is I need It in Max 3-4 Days. This Project is Developed in Core PHP, No Frame Work Requirements are 1. We are Indian Company. So We Need India as a Default Country, So This Button should work like “Outside India”. I want to Write City and State whatever I want to write (Doesn't Want to Select State). And When we select this button then all the country list should apear. ([login to view URL]) Ref Image Below 2. Now I want a Date Picker, Admin or User who click on Follow Up of an Account should also mark a Date, When to Follow. ([login to view URL]) Ref Image Below 3. Now here I want to see the Date of Follow. I must See when I have to Follow the account. I should know on which date I have to call the account. Date should be in DD/MM/YY Format. I also Want a Button with which I can see the details or basic information of the account like Name, Phone Number, Other Imp Details or Everything I have Mentioned on creating a new account. I should know In Which date I have to follow how many accounts. And I also want to Export or Print this report. My Client want to see the report in print format too, to see how many accounts to be follow up in any particular date. This Report can be exported or viewed on screen by selecting the Starting and Ending Date.
