Hello everybody, I have previously posted this project: https://www.freelancer.com/jobs/php/Create-online-form-save-PDF/ Due to personal reasons, the freelancer who that project had been awarded to was unable to complete it. However, he has provided me with all the work he had done so far - HTML and CSS for the forms are done (with just a few modifications to make), as well as part of the JS and validation. You can see the detailed field requirements in the attached file and I can provide all the work that has been done so far. I've also attached a file describing how the form should work, step-by-step.
