Wordpress - Conditional Logic - Cost Estimation Form

HI We have an existing Wordpress site and would like to add a Cost Estimation Form. The cost estimation form would then be attached to WooCommerce allowing the user to checkout after getting the estimate. Here is the example of the form:- [login to view URL] We currently have the following plugin to do the job. You can tweak it to add the multiplication features. The plugin is already attached to WooCommerce. We don't believe in reinventing the wheel so any other additional plugins are welcomed. [login to view URL] We want it done in the most minimalistic manner so rest of our plugins are not effected. Please only quote if you can deliver the job in the above described way.
