Wordpress custom post type- permalink rewrite using Meta

Looking to make a custom post type prepend the slug with the date selected from a datepicker inside the post. Most of this is already functioning (the permalink setup works when you create a new post, but there is a 404 error when you try to view the post). Most likely the fix will be related to initializing the proper rewrite tags, since what we are trying to do is not native tags to wordpress. Should be a quick fix.
