Web form possibly in acrobat

I need to have? an application form that can be completed online by users? and then sent as a whole complete? form to an email address. I already have the form written in adobe with the active fields, but I can't find a way of being able to send this form complete. I don't really? want just the field content sent, Ideally the whole form so that it can be printed out by the receiver. ## Deliverables A copy of the form in PDF can be found at [login to view URL] I will also require a sample version to ensure you understand what I require, this only need to have a couple of fields and posted on here so I can test on my server, or if you have your own hosting you can put it there for me to test, remember the end result must be an emailed filled out PDF form. I will give you a test email address to put into the form so it sends the completed form to the right place.
