Custom Multi Step Contact/Submit Form

Hello, We are looking to hire a PHP developer to create and implement a custom contact/submission form for us. This form should do the following: 1. Allow a person to fill out a form. 2. Once the form is filled out they are taking to part two where they are allowed to upgrade their submission. 3. They can choose to upgrade or not. If they choose not to, they should be taken to a submission success page. If they wish to upgrade their submission, they should see a list of upgrade options (which I will provide). These different upgrade options will have unique prices. A person could choose more than one option, and there should be a "total" section somewhere that automatically sums the total of their upgrade order. 4. Once the the upgrade options are selected, the person should have the action to purchase them and be taken to a Paypal Payment page. 5. Once the payment is made and verified, the user should be brought back to a confirmation page. 6. An email should be sent out to us and the user to confirm the submission that was made.
