Shareware Nag Screen Need Making - Very Simple!

Hi, I have a demo version of a program I sell which has a nag screen in it. The nag screen pops up when the user exits the program or clicks the "Buy" button in the software. On it, it lists the reasons why you should buy the full version and how much it costs etc. It also allows you to pay though PayPal or Credit/Debit Card. However after I changed the nag screen on my program I got a lot less sales. I need someone to make a much more attention grabing nag screen so more people would want to buy the software. It MUST be made in Visual Basic 6 and will only need to be around 1 to 3 forms in size. The nag screen should be colorfull and keep in the color scheme of the program, it should allow the user to pay though paypal or credit/debit card (it just needs to link the the payment pages I already have) If the user chooses paypal they should be able to pick the currency (links provided).
