PHP Form to Database + Fasttransact Payment Processing

**General Info PHP/MySQL Only **Visitors will be able to fill out a form to register for a race. Form will collect 40 different things. Visitors need ability to login, or create a new account by filling out this form. If they login, this form will be pre-populated with their previous answers. Data needs to be saved to database. The visitor will then be taken to a second form where they will enter there Billing Address and Credit Card or Check data. The billing name and billing address should be populated with the information they entered on the first form. The billing name and address needs saved to the database separately in case it's different. The credit card or online check data will NOT be saved to the database, but will be securely posted to a FastTransact Merchant Account for verification. The merchant will reply with Approved or Declined and the response should be saved to the database. **Backend/Admin **Add or Remove Racers Edit Racers Information Change Payment Status to Paid or Not Paid Each year there will be a new race, and the admin area should have a place where I can set what year registrants are registering for when they fill out the forms. I need to have reporting so I can View: Paid Registrants by Year Unpaid Registrants by Year I need to have a function to email: Paid Registrants by Year Unpaid Registrants by Year All registrants... Paid or Unpaid. Send with POP3 in batches. X amount of Emails every X amount of time. (So I don't go over my hostings limit of emails per hour/day.) A CVS file of all registrants needs to be sent to me once per night. For every Approved Payment, the form data should be sent as a PDF to me. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
