Convert PDF to Electronic Form

To reduce environmental waste I am attempting to run a paper-free, health and wellness business. I currently use an intake form (single page) to collect client data and health history. I would like to have the ability to either email clients an electronic form to fill out prior to coming in or have them sit in my lobby and fill out the form using a dedicated iPad.
(NOTE: It would be great if clients could also mark on the body image where their symptoms are located.)

Hi How about using a Google Form? They can fill in after you send them a link or they can use an ipad to do this in your office.
