Coder for a Simple web update job PHP

I need a coder who have knowledge of PHP to do up a simple job. My client needs his website to be updated: 1) Remove and Add in new fields (about 5-7 fields) in the résumé submit form ??" Job applied, Address 1, Address 2, City, etc 2) Append the details from the fields above to become the 1st page of the résumé (from the Copy and Paste CV field) to be sent to the office clerk, in doc format. 3) Add in 1 new web page based on existing web pages’ design, basically copy, paste and modify. You will be working on only 1 page which is the existing CV/Résumé submit form and add in 1 new page. Should take about a few hours to complete the entire tasks. Clear instructions (in writing and screenshots) will be provided to the selected coder who won the bid. Kindly let me know the duration in number of HOURS to complete the job. Looking for someone who is reliable and can communicate well with me, for upcoming future projects also. I like someone who is honest and not afraid to ask me questions to clarify things. Strongly prefer someone within the GMT+7 -GMT+8 timezone so that we can communicate real time via webcam and online chat and voice chat. Consider this as a test for you to see if we can work together for long term 3-5 years, whether we both are comfortable working with each other. ## Deliverables 1) All deliverables will be considered "work made for hire" under U.S. Copyright law. Employer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the employer on the site per the worker's Worker Legal Agreement). 2) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 3) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Employer's environment--Deliverables must be installed by the Worker in ready-to-run condition in the Employer's environment. b) For all others including desktop software or software the employer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this project. ## Platform HTML, CSS, PHP
