Convert /program 100 page PDF to FILLABLE PDF

I have a 100-page PDF document that needs to be programmed into a fillable PDF format. There are 17 pages that require no fillable PDF programming. I will supply the PDF upon acceptance of the bid. This needs to be completed within one week of acceptance of a bid. So, if you cannot meet that timeframe for this project, please do not bid. The majority of the pages are forms that have been created. Many of the pages are repeats of each other (as noted in the extended description). I would like to have someone that has extended experience in creating fillable PDFs. So, please not this experience in your bid. If you can provide examples of your previous work, that is even better. I will be looking closely at this in deciding whom to accept a bid from. ## Deliverables **Information on the PDF itself:** A person needs to be able to save the PDF with different files names so that several different ones can be created (for each family member, as an example) When someone fills in the Name section on any of the form pages (starting page 11 and thereafter, it should automatically fill in the Name on all the other form pages: 11-52). Pages 1-9 have no fillable PDF information that needs programmed. Page 10 is a personal contact information page. The Additional Comments section should type from one line to the next when someone comes to the end of a line and is typing. Page 11 has four sections of the exact same form. These should be programmed the same way. However, they should NOT fill in all sections when one section is filled out. Pages 12-15 are the exact same page- each with four sections that are the exact same form. These should all be programmed the same way. However, they should NOT fill in all sections with the same information when one section is filled out. They are meant to be filled in separately. Page 16 has four sections of the exact same form. These should be programmed the same way. However, they should NOT fill in all sections when one section is filled out. Page 17 has two sections of the exact same form. These should be programmed the same way. However, they should NOT fill in all sections when one section is filled out. Page 18 has four sections of the exact same form. These should be programmed the same way. However, they should NOT fill in all sections when one section is filled out. Pages 19 - 29 are the exact same page- each with three sections that are the exact same form. These should all be programmed the same way. However, they should NOT fill in all sections with the same information when one section is filled out. They are meant to be filled in separately. Pages 30 -32 are the exact same page- each with four sections that are the exact same form. These should all be programmed the same way. However, they should NOT fill in all sections with the same information when one section is filled out. They are meant to be filled in separately. Pages 33-34 are the exact same page- each with seven sections that are the exact same form. These should all be programmed the same way. However, they should NOT fill in all sections with the same information when one section is filled out. They are meant to be filled in separately. Pages 35-38 are the exact same page- each with four sections that are the exact same form. These should all be programmed the same way. However, they should NOT fill in all sections with the same information when one section is filled out. They are meant to be filled in separately. The yes and no boxes should be able to be check marked. Pages 39- 42 are the exact same page- each with four sections that are the exact same form. These should all be programmed the same way. However, they should NOT fill in all sections with the same information when one section is filled out. They are meant to be filled in separately. Pages 43 - 48 are the exact same page- each with six sections that are the exact same form. These should all be programmed the same way. However, they should NOT fill in all sections with the same information when one section is filled out. They are meant to be filled in separately. Pages 49- 50 is a two page form that is exactly the same as pages 51-52 and they both should be programmed in the exact same way. The yes and no boxes should be able to be check marked. Page 53 has no fillable PDF information that needs programmed. Pages 54-89 are full-page journal pages. They should flow from one to another when a person is typing/filling them out. This means that if a person types a full page, when it gets to the end of the page, it should automatically continue typing on the next page. If they reach the end of the their journal pages, it should just stop. These pages should have a ? inch border and basically just be able to be typed on like a blank page- below the header and above the footer. The fillable PDF should be programmed to be able to add additional journal pages. This assumes they have the proper software and know how to extract, add pages, etc. Pages 90-93 have no fillable PDF information that needs programmed. Pages 94-97 are a four-page form. There are several questions that have a box the needs to be able to be check marked when filling it in. The last comment section should type from one line to the next when someone comes to the end of a line and is typing. Pages 98-100 have no fillable PDF information that needs programmed.
