The form must have the following attributes: - Controls like, radio buttons, text boxes and multi-line text boxes. - Should work on mobile devices without the need of another software to be installed (We are assuming Android and iOS. If there are more devices that needs to be supported, please let us know) - There will be some calculations done based on the data user added. - Must not be editable by the clients Once the form has been filled and saved by the clients, the data will be sent to you. This can be achieved using the following ways: - Each submission will be sent to you via email - Submissions will be saved online and you can access those using an online interface.
