i require a website which allows the users to do the following.
Complete a comprehensive questionnaire.
From that questionnaire information will be transferred to approximately 20 different forms, and should the individual be eligible (each form has different requirements to meet) the relevant forms will need to be emailed, faxed, or printed out to different departments.
There will be upto 150 fields in the initial questionnaire.
Once the site has been tested and is working correctly we will start work on phase two which is replicating the system to different phone apps. The apps must be linked to the main site so that one update, such as a change of form will make the relevant changes throughout the applications.
