Chrono Form - Formatting - Quick Turnaround

I need a template applied to a Joomla Chrono form.

Using this form design as an example:

Using the attached Chrono form elements apply the new look given the example. When I say new look I am referring too what I pointed out in the [url removed, login to view] file.

Note: Only the elements in the supplied Chrono form need to be used.

This should be a quick job, no more than 2 hours.

Job will pay $30 covering any tweaks that may be needed after inital review.

I would like this completed in the next 8 hours.
