High expert Prestashop - creating a form using module

Hi ! URGENT PROJECT We are a small webagency and very very often need Prestashop developpers . Here is 1 URGENT URGENT project for our client : MISSION : Prestashop creating a form using module -->module account group par netcodes, see module here : [login to view URL], or you can see on addons Prestashop website -->Module is already installed but client do not know to use it -->we provide simple design DEADLINE : 2 days (this week-end) REQUIREMENTS (we do not answer if you do not check and confirm this) : 1) You are available 6 hours a day 2) You are free and have not any other project 3) Everyday you will give us a report of progress, if 1 day we have no report, we abort project 4) What websites have you done on Prestashop ?
