Need an expert in php and html to help me fix a contact form and some nav bar issue in chrome

I am having issues with contact form, I have created [login to view URL] and form some reason when submit is pressed it goes to a white page there is no way to check php error logs as I am on shared hosting, I am also having issue with url fragment not working correctly on chrome the first click is fine after that the nav bar does not respond to clicks, I need these two issues resolved anyone with good understanding of PHP and HTML would be able to solve this within minutes.
