Load data into modal and then submit

Modal should be based on [login to view URL] version 0.100.0 and jQuery (JavaScript is also acceptibel) should be used for loading and submitting data. I need a modal that loads data from a php file based on passed on attribute value. The data should be loaded into a form element and then at the end there should be an option to submit. Example. <ul> <li><a href="#" id="1">Item 1</a></li> <li><a href="#" id="2">Item 2</a></li> <ul> If you click on any of the items. It will pass id value to a .php file and the php file will return data based on id value into a modal. Once the data is loaded into a form modal there should be an option to submit which will post the data to another .php file. Modal will update and show success/error message.
