Custom Web based software

I want to develop a simple web based software which will need around 7 masters, 2 webservices, 2-3 transaction form and around 3 simple reports. I prefer the software in open source platform like PHP/LAMP. The application is simple database application and do not have much complexity. I like the project to be completed in max 30-45 days so team or person who can complete in time need to apply. I personally feel its not that big an effort. The team will need to coordinate with another team who has done the hardware design part for integration testing. I will need documentation for the projects so I can maintain the same which should include database design, pseudo code for application flow, source code with documentation, test cases used for testing, user documentation for installation and using the software. I can explain details after signing NDA with the developer. I like to see some software layout as will like the graphics layout to be good looking and impressive and part of the solution. I like developer not to sell the software done to any other provider.
