I have been removing malware and injected code from a lot of sites recently. With Wordpress the best option is to do a manual upgrade to 4.1.1 and replace all of the core files. Then, scan the theme and plugin files for issues. I will most likely have to manually make some of the changes. I should be able to have this done for you in a few hours. Happy to help.
