Build PHP/MySQL login and CRUD functionality into existing website and MySQL database

Here's what I'd like to do:
1. If you go to [url removed, login to view], there is a Login button at the top. I created a modal popup with login/register tabs. This worked at one point, however it does not pop up for some reason right now. I would like to fix this so it pops up - this is probably just a simple JS fix but I'm not sure what's causing the error.
2. The login/register functionality should tie to a MySQL database which I've already set up. I believe I had the login functionality working but not the registration. I would like to ensure both forms work and produce appropriate errors when wrong data is supplied (errors should be displayed within the modal).
3. When a user has successfully logged in, they should be directed to a new page with a special navbar at the top that cannot be viewed without logging in.
4. I would like a simple PHP/MySQL template for displaying data in a grid format with the ability to add/edit/delete (CRUD). Ideally I'd like to be able to do this in-line, however I've set up a basic sample of what I'm looking for using PHPGrid Lite here: [url removed, login to view] I'd like something that does not require a paid 3rd party service and can be easily replicated.
