Website Design (3 pages)

Ok, I will be selling websites in my local area and will then be passing them on to freelancers internationally. For my web design business I am in need of a clean easy to navigate website with some easy colours (Lime green, light blue). I will need 3 pages. I front page, contact page, and a content page. I would like the front page to have a flash banner indicating the process of ordering a web design. And images which will show the prices and packages that my client has to choose from (editable via PSD). On the contact page I need a PHP form that will send the details to my desired email address. For the content page I would like a blank page where I can fill in my own content. I am looking to pay 200 USD for this project. Please reply with your Portfolio, Name, Contact details, previous experience and any additional info that I may have forgotten. I may have future projects for the freelancer if the service is good.
