Password entry webpage

I require a two step process that needs a simple password protected webpage that requires a visitor to enter their name and a password that I specify before they are taken to the main sales page. On the main sales page I require a form and an image of a check/cheque that when numbers are entered in a box the check/cheque will show the annual sales figures i pounds or dollars. A great example of what I need is shown on this website: **[login to view URL] password: xx33567gb ** A clone of this would be great I would need to have some control over the cost for items sold, the password, and the name on the check/cheque would be as entered by the visitor on the first web page. The date on the check/cheque would need to be the current date. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
