Questionnaire project

I need to find a serious php/mysql specialist to finish a customizable questionnaire that has been implemented in my web application [login to view URL] by a programmer who did not finished his job properly. The site works with a mysql database. Athletes and teams are stored in separate tables (team_id and user_id available). I have attached a script containing the structure of the users, teams (tbl_group) and questionnaire tables. Also attached, the code for the questionnaire modules. What needs to be done : 1) Implementation of a questionnaire administrative control panel that permits to manage questionnaire creation/delete/update more easily (all the administrative functions are already functional) DYNAMIC CHARTS (see attached XLS file for more details) 1) Group charts a) Need to display one chart that includes all players results (the average score). The chart should average questionnaire results for each athlete between the 2 dates and display the resulting values. In order to quickly identify tired athletes, a red vertical line must be added to illustrate the average threshold value (based on average threshold value of all the questions of the selected questionnaire). b) Being able to display a data table who show detailed the questionnaire global results -not average- for all the athletes between the 2 dates. 2) Individual charts: a) Needs a chart to present all athletes average scores results on the same graph (see attached Excel file for samples). b) Being able to display a table that show detailed data (ie: questionnaire raw results) for the selected athlete between 2 specific dates as well as for today’s date c) I also need to be able to display detailed questionnaire results (based on each questionnaire category result) for a specific date as well as today’s date.
