564052 small php / css website changes 2

In a website of one of our client we need two small changes:

- Logo on the upper left should be linkable to startpage

- Form that is in the website containing 4 fields should actually send e-mail out (now there is nothing behind)

Find attached the entire site as zipfile.

Looking for quick project in selecting, programming and payment since these are small changes for an expert.
