Write an iPhone application for a technician to enter a work order -- 2

The app needs to be native iOS and look like a modern app with our company logo integrated in a non-disruptive but classy fashion. I'll also need instructions on how to get this installed on our corporate iPhones. The use case for this app is for a technician to enter in work performed and have it sent to a central system where we can bill for the work. The users are technicians working on forklifts (or similar equipment) so we need to make this as simple for them as possible. The layout needs to be clean, easy to operate and easy to fix mistakes. This is the workflow: 1. Tech opens the application on iPhone 2. A menu of items appear. These should include "Create New Work Order", "Edit Saved Work Order", "Lookup Asset Information", "Calendar", "Parts Inventory". The last 3 items can just show a screen that says Not Implemented. 3. Tech selects Create New Work Order and points the camera at a QR sticker on the forklift and the app decodes the QR and types the value in an asset # text box (or he can type it in manually if needed) 4. Tech picks a Resolution code from a pick list of about 10 items (i.e. Abuse, Contract Maintenance, Fuel, Parts, etc.) 5. Tech picks a Level 1 Service Code from a static list of 10 items (Tires/Wheels, Electrical, Brakes, etc.) 6. Tech picks a Level 2 Sub-Service Code from a static list (which is limited based on the selection from above but about 10 items maximum) 7. Tech enters the hour meter reading (typically some number between 0 and 30000) 8. Tech enters the amount of hours spent on the repair using +/- buttons in 15 minute increments. The default is 1 hour. 9. A text description of service performed needs to be provided. A text box should take up the screen where the Tech clicks Record and speaks at the phone and Siri types in whatever he says. The Tech can edit this afterword if needed and there should be a a key to wipe it out completely and start over. 10. (optional) Tech clicks Parts and points the camera at a part barcode which gets added to the parts used list (or types in manually). He can do this several times if there are multiple parts. 11. (optional) Tech clicks Photo and camera apps pops up and he can take a photo. He can do this several times. 12. Tech clicks done and sees a summary of the work order. If it's correct he can send the work order data along with a configurable site code (there will be about 200 techs using this at 150 different locations so we need to know which site this is from) magically zips off to a table in Salesforce where we can view it or import it into another system. 13. At any time the Tech can click "Save" and it should save the work in progress and bring him back to the main menu. If this project is successfully adopted by the technicians I'll be adding additional functionality to the program with additional menu items.
