Simple take-picture and upload-app (app-store)

I have a website that lets a user manually add inventory-items (for instance stamps, paintings or other belongings for documentation) for insurance or other reasons: https://inventarlisten.no. Should be an iPhone/iPad-app.

I want an app that will will allow user to start the app, get the screen for taking a picture of an item (optionally let the user give the object a short name) and then do a simple post (stream) with the content to a script (.php) that will accept the post-request of the picture+the optional name (you don't have to write the PHP-script).

Functionality:

- On first start-up, it should ask for username (email-address) and password - will be checked against our PHP-script on website and our website will return a hash-ID. This ID will be used for all later posts to the website.

- From now on, when user starts app, it should start with showing our logo + showing a camera window to let the user take a picture. As soon as user has taken picture, show a text-box (optional) for typing a short name and let user choose Upload or Trash. When Upload, the image + optional name + hash-ID is sent to our PHP-script.

App should be very simple, with our small logo present on load or maybe permanent. It needs to only store the hash-ID. It doesn't need to show any previously taken picture.

So just take Take picture->upload, Take picture-upload, Take picture-upload... and so on :) If an upload fails for any reason, user should be able to retry. That's it. No menus etc. needed I think. Maybe ask for username/pass if many attemtps to login fails. Should be easy to repeat the process for many items. One at a time.
