I am creating a new website that will require a secure survey built into the site. The database I want to use is MySQL. The server will be running Ubuntu 16.04. I do NOT want to use Survey Monkey or anything like that. The program must be written in python (back-end; javascript for the front-end). There is publicly available python and javascript code that will do most of what I want (with modification), but I don’t have the time to tinker with the code and do this myself. The program should do the following:
The user will click a button to launch the survey. The user will get a screen, which requires entry of their name and email address. They will click next. The system will send an authentication code to their email address for verification along with a unique identifier. They must enter this code on the next page and press verify. Once the code has been entered successfully, they are presented with the survey.
The survey will consist of 50 questions. The first question requires entry of a number from zero to four with two digits after the decimal point. The second question requires entry of a percentage from 0% to 100%. The remaining 48 questions are statements, followed by a 5-point likert-scale (strongly disagree = 1, disagree=2, neutral=3, agree=4, strongly agree=5).
When the user completes the survey, they click submit and the system will store in the MySQL database the user’s name, the unique identifier, their email address, date and time, the responses to all fifty questions and an index score. The index score is a number from 0 to 100 that the system calculates based on the answers to the fifty questions using an algorithm I can give you privately (it is not difficult to code).
The user is then sent an email with their index score and a series of recommendations based on the index score and the responses to all fifty questions. In other words, the email will need to be constructed programmably based on the survey responses using another algorithm.
The user may take this survey two or three times per year over the next four years and the system will need to store all records and not replace previous ones. (I am doing a time series research project).

It's like NPS and ENPS, and i am familiar with this procedure, recently i done it for a client and it was a 10-point likert-scale after that we calculated with algorithm and develop a pie chart for further use.

I write this bid proposal after reviewing and understand your requirements that you like to build a website where admin (you) able to add/create the survey for the end users and add the question and set their marks in reference of 100. First 2 question will be fixed and the input type will be the textbox and support the numeric value only and the user would able to enter the value in decimal also.
The end user/visitor would able to come and register on the website by entering the mobile number, name, and email id. The system generated code will send on the email for the email verification. The user needs to enter the code on the website for the verification process. Now, the user would able to check the question one by one and enter his answer. First two question will support the numeric values and rest will have the radio button with there options. Once the user submits his survey they will not have any write to backtrack and make the changes. All the questions will save in the database along with the username, his email and their options. Based on their submitted option the algorithm will calculate the marks and display on the screen.

With storing and saving of result or rest part i have no issue .

Is you server have email server (smtp) or you were going to use third party service to send mail.

Hi, are you looking to do a personality test instead of a survey?
I've developed a similar questionnaire for MBTI-16PF for another client, using PHP instead of Python. Same can be developed in Python as well, using flask for routing, although PHP is recommended due to its better web capabilities, unless you have a specific reason to select python.
In either case, the requisite functionality can be built in 5 working days. Further, if you need, we can also create a PDF report for the recommendations and mail that instead of plain recommendation mail. Although, will need to provide for additional budget for that.
Let me know if you would like to see a sample report that was created for a similar but high budget project which involved multilingual PDF report (English and Greek).
