convert 10 PDF printable forms to a web application which users can fill out the forms online

This project is to convert these 10 PDF printable forms to a web application which users can fill out the forms online. There will have to be a back-end in which the owner can administrate end-users (defendants), as well as edit/delete the submitted forms and accounts. In addition, there will have to be a front-end for the owner + employees to view, sort, and print the forms. Finally, there needs to be a user system for the owner so he can assign accounts to his employees to manager the submitted forms. so there should be one admin account that can administrate the employee users and the forms themselves, and a normal level account that can only administrate the forms. Job duties are: ******Must review all files and layouts - Entire application will be password protected. There should be two different login portals: - 1 for admins with two types of users - Admin: Can create other users and manage submitted forms as well as end-users - Employee: Can Manage submitted forms as well as end-users - Another for the end-users that are to fill out the forms. - These users will be created by the two admins (Admin or Employee). End users should not be able to create their own account. - There Will be a couple fields that will be universal to all forms for end-users. These fields will be required to be entered by admins when creating end-user accounts. These fields should auto-populate in the forms where they appear: - Name - Bond Number - Agent - Bail amount - City - state - county - Convert all forms to html web forms to be stored in a MySQL Database using PHP to process data. - The web page structure/design should closely resemble the appropriate form. No HTML tables should be used. - Should have validation for empty fields, phone numbers, and email addresses. There should be an easy way to make fields required or not (in the programming, does not have to be part of the interface). - Data to be stored in a MySQL database. SQL markup to create database and tables should come with the final source code. - Any where a Social Security Number is requested should be "Last 4 digits of Social Security Number." - Send an email alert when a set of forms is completed. - any signature field should still be signature but the user will just type there name. - The Forms stored in the database should relate to user that they belong to. - Users should be able fill out forms one at a time. There should be an indication in the navigation that a form is completed or not (such as a green check or red x). - Once submitted, Admins should have an interface to view and search records: - View all in alphabetical order (By last name) - Display Records by first letter of last name - Display records by bond agent - Display records by state - Search - Search By Name - Search by Bond Number
