WANTED: BootStrap Rockstar Front-end Designer for a SaaS service

Hello, How are you? I hope this finds you well. I'm looking for a Bootstrap Rockstar to design a fabulous form by enhancing an existing web form for an upcoming SaaS service. I'm looking for an A+ quality designer/developer, and you must produce work with precision and high quality. I do not have the time to train nor the patience for losers and time-wasters. This is a serious project, and I'm looking to hire the right person ASAP. In the attached zip file you will find three files where [login to view URL] is how the existing form looks like and needs to be converted to a user-friendly interface that uses panels/accordions similar to [login to view URL] and FB2.jpg. You can use the basic Bootstrap V3 design template. Once you complete the form, I will go ahead and hire you for completion of the rest of the pages. The budget value placed here is just a placeholder. Actual budget is pretty flexible depending upon the quality and the turnaround time.
