I have a simple form i need to post on mturk which will have some dynamic data fields and I just cant seem to figure out whats wrong with my markup so I would like to hire someone to generate it and then post to me the final and i will test and run it. here is my survey . [login to view URL] you can tell the dynamic fields with the $() and the final input that we want from the user. Please make sure when you test this that it actually works and needs a csv to be uploaded to it. This is the part i keep failing on on my side. Posts on this that do not talk about mturk will be downvoted and deleted. The generic posts are a big waste of time for you and me :)
