I need an optin or squeezepage put onto my website - 23/07/2018 22:37 EDT

Ive just completed my website, and I can't put an optin box on my site to gain subscribers. All I need is their name and email address in exchange for a free 2 week meal plan. Also is there an option to change the free gift after a certain time? My site is about nutrition in helping people who wish to switch to veganism or go vegetarian. I haven't hyet created my 2 week meal plan, which won't take long. Im not sure what else I can actually tell you at thisn point. Please email me for more info on my site as it also covers health diseases, articles, reports, of which I haven't done as yet, I thought I should get this optin box up and going first. Ive uploaded my blog header, this is the same as my FB cover photo too. Thanks so much.
