Need a form redesigned

We need a form redesigned to allow room for an "click here to upload" field. My client uses RightSignature, which allows us to place a clickable field on a PDF to prompt an upload. This is a new feature, so there's no logical place to place this function in the current form.

So basically, this new form would still need to fit on one page, include everything on the current version, and have a clear area for "click here to upload attachments". It does not need to be beautiful or artistic - just legible.
