Build out a complex lead form using a platform.
- Approx 5-12 fields
- Needs to replicate an existing lead form (will provide an example).
- Has conditional logic 1-2 splits in logic. Platform will allow logic splits (turnkey).
- Need someone to draft it up, test it, then show it to me.
- UX needs to be kept in mind also.

I will approach in 2 parts. 1 is initial to get it to a point where I can test it, part 2 is when any feedback chnages are made and form works and looks as intended
