I need an accountability Excel spreadsheet created. This spreadsheet will be used to track employee quality errors and ultimately provide to the supervisor what action to take.
Two different types of errors to track: Minor Errors and Escalated Errors.
1 Escalated error within first 30 days of hire date, freebie.
1 Escalated error after first 30 days of hire date, level 1 corrective action.
2 Escalated errors after first 30 days of hire date, level 2 corrective action.
3 Escalated errors after first 30 days of hire date, termination.
6 Minor errors after first 30 days of hire date and within a 6 month period, level 1 corrective action
8 Minor errors after first 30 days of hire date and within a 6 month period, level 2 corrective action
10 Minor errors after first 30 days of hire date and within a 6 month period, level 3/final corrective action
12 Minor errors after first 30 days of hire date and within a 6 month period, termination

This is very similar to the Quality forms and reports that I used to prepare for the Quality Monitoring Team. I was a dedicated resource for this purpose. Hence I have experience in preparing this kind of spreadsheet.
