Build an app and website

I need an app that will be a large survey with many many questions in different categories. (example: How many hours do you watch TV per day? A. 1 hour b. 2 hours c. 3 hours etc....) each question will have up to 10 possible different answers. I want the results of each question to be displayed in colourful graphs. Bar/pie charts. The surveys will be ongoing so the graphs will have to continuously be changing and displaying the most current results. The app will also be a website which will need to have individual user accounts where people can log in and keep track of surveys they have taken and follow the ones they have completed as well as a forum. If this is something you feel you are capable of please contact with examples of past work. Your estimated cost and time to complete. This must look good and work well.
