Hello we need an electronic waiver form designed and implemented (we have the content) I will need to: 1 Be able to take data from fields already in our website. I.e name, address email, phone, etc and enter them into the waiver form automatically. 2 it will need to have checkboxes on the way to acknowledge that they have read certain parts of the waiver (see attached) 3 They will need to sign at the end with our signature pad 4 it will also prompt to take a photo with our computer camera 5 after they sign and take photo we will have to store either on or server or google drive, dropbox or alike

Hello Sir/Maddam. I would love to work on this job for you. First I would need to confirm that this form is intended to be displayed on a website (as apposed to a mobile app or other software). If so, the steps I'd need to take in order to understand your requirements and deliver on this project would be: 1. Find out what back-end you are using and where the required fields exist in your database 2. Discuss what you mean by 'signature pad', and 3.
