Making a database with Microsoft Access that has a data entry form & report

Hello everyone, I am in need for someone to make me a database through Microsoft Access and in this database also include a data entry form & report in the database as well. I will provide you the Microsoft Access file that I need the database made through and the instructions but I need you too add forms, test data, queries and report. Other then that I need this done by 12/17/2016 by 9:00pm which is Saturday Central time or latest 12/18/2016 at 11:00 am Central Time. If you have any questions, please let me know!
