Create file from a excel list of categories

We have a website that needs a the list of categories we have the excel sheet of all categories in about 8 or 9 different language we need someone to create one of either of the three (CVS,SQL,CVS using LOAD DATA) files for each languages you will need to get the contents from our current database and convert the excel spreadsheet to make the content to work you will have to make sure it will work you might have to create ID's, it will have to work so I still can get the info in our admin panel.
we need to make sure that the file is right with all the database contents so it will work right I want it to be where I can import each language as I need it
(((((I will attached samples so you can see what we need )))))
you will see our current database has categories now but we know that all the categories don't have description
but we will worry about that later right now we just need all the categories from the excel sheet so we can import them in our database
