Design/Artwork on a website mockup, few hours for the right designer

Hello, i could do with some help please.
I have a website design mock up. I'd like to work with someone on it for a few hours to improve it.
Please note, this is a PDF, not layered PSD.
To help with bidding:
1. It includes a banner with a form.
2. I would like the banner exchanged please with something relevant from the Istock or SHutterstock examples I will send a link to
3. There are two scrolling icons on the banner, can these be changed so that they match.
4. Can we put some colour onto the consultation form
5. There are 6 images, can these be swapped for relevant ones from the istock/shutterstock examples.
6. In the footer, can we mock up the latest news. Ie, news item 1, news item 2, news item 3. Can these then be moved to the right of the footer.
7. In the middle of the footer, can we add a map please. The address is will be provided
8. Re. the above address, if you would like to design this in so it fits nicely with the black background of the footer that would be great.
9. Anything else you think will improve it.
I'm looking for an experienced designer to spend a few hours on getting this mock up right.
