Custom Web Invoicing Page

The attached file must become a calculable web-based form, which generates a saveable PDF of itself once filled out. Certain line items must become minimum amounts regardless of what the costs entered state, although the costs entered must remain displayed (see attached file). The option to email the generated PDF is also desired. Notes: - The PDF should print properly on 8.5x11 paper - I should be able to edit the text/images/layout of the form page and have the generated PDF reflect these changes. - A client-side 'live' calculation of the fields must occur for ease of use without reloading the page - The form itself does not need to include the fine print and non-invoice/referral information, but the generated PDF must replicate the attached file identically.
