Convert 4 PDF pages to HTML

We have 3 forms - 1 in pdf format and 2 in word documents that we need convering into html format.
They are order forms with fairly simple boxes and a logo. Each one is an A4 page.
We need the forms in html to look exactly as they do in the pdf or Word format.
Information collected from a website will be transposed onto these forms and emailed to us where it will then be faxed/fowarded to a dispatch warehouse.
Our web programmer will insert all the relevant coding for the information collected from the site to be entered afterwards.
## Deliverables
1) 3 Completed forms in html format looking exactly like the original forms in pdf and word, complete with wording, boxes and logos.
