Creating a Report for a Wordpress PHP site

I need code written to pull data out of MySQL database and display it on a Wordpress page as a report. This means we need a page where the user can select their search criteria and the final report will need to display a certain number of results per page with "next" and "previous" buttons so users can review all of the data. The wordpress website is already built. The data is in an excel spreadsheet and it needs to be loaded into my database first, therefore new tables will need to be created in the database. Please show me a project where you have done this type of work before. I've attached a copy of the file that will need to be imported. All work must be properly documented.
