Component to Retrieve Outlook AddressBook OR .csv

I am looking for a utility in which there will be a few screens that will guide the user through importing from an outlook address book or .csv file address book. I am attaching my database structure in which I would like to populate. Either from outlook or .csv, the user should be able to map the fields from the source file fields to the destination fields. If there is no target field, the information should be stored in the otherinformation field as a name? value pair.? This will take a few tries until the screens are appropriate and easy to use for end users. Usability must be considered. The utility should also be able to export in such a fashion that outlook & outlook express should be able to import the contact information. We may need to do some prototyping, then finalizing the screens. This piece will fit into a larger application that contains a contact addressbook, which I have already written. I just need the piece to import/export from outlook/outlook express to/from my database.
