Create Acrobat Forms

I need someone who is familiar with Adobe Acrobat forms to create a form that can be part of a PDF catalog where the customer can fill in the form included in/with the integrated catalog - The form would be emailed back so that the order can be filled - Payment by PayPal, Check or Money Order - Associated software to potentially automate the operation would be desireable - The catalog would reside on a website location for free download and could also be sent as an attachement to an email or as a digital catalog mailed on a USB drive - Would need to discuss further before making a commitment
