Create an online application form

I have an application form in PDF which needs to be ported online. I will attach the application. It should function just like this application <[login to view URL]> plus i need it be saveable, that is the user can fill out the application incrementally and save the state. Once the the application is complete, the user can submit application. There will be 2 buttons. "Save" and "Save&Submit". The form can be saved&submitted once all the required fields are filled in and valid. At any point during filling out the application, it can be saved by pressing Save button. That will save whatever fields applicant already filled in in the database. The user can return to saved application by typing in application ID (8-10 symbol generated string) and family name and contunue filling out the information. Note that the fotos should be saved as well. So, if user closes browser and then reopens it, after providing application ID and last name, it will return to the state the application was before. If there are any fotos that were uploaded, they will be in saved application, and user can change the fotos if he/she needs. Once the application is filled out, the user presses Save&submit. That will persist the data to database, send out email and mark the application as submitted so that the applicants cannot make changes to it.
