Create a Joomla Module for product code creation

We need a Joomla module form that can build a product number based on selected tick boxes, once the product ID has been created it will need to link to the product in the store. This module will need to be customizable by us in the backend of joomla, if we need to add or change a product number or size.
Please see attached .jpg with a diagram of the module, as customers select the sizes of there specific part the number gets created at the bottom of the page, this number will then link to the part in the online store once complete.
The attached .pdf is one of the Identification forms we will need, there are a few more so we will need to be able to create our own ones in the future from this module.
