Contact us & Inquiry form (PHP for Google Docs)

Hi there, We require some PHP code written for a "Product Inquiry" form that will integrate with a google documents spreadsheet. We will be asking clients to submit information using our form, that will be added to our google documents spreadsheet. We already use Google's embedded HTML form, but we cannot edit the styling and prefer to have the form called using a PHP include command to remove our google documents code from the page source code. The data that we require on the form is: Full Name: (text box) *required field Company: (text box) Product: (list - 13 options in length... "product 1, product 2, ... product 13") *required field Inquiry Regarding: (list - 8 options in length... "option 1, option 2, ... option 8") *required field Please respond to my inquiry by: (radio button - options "phone, email") *required field City, State, Country: (text box) Phone: (text box)*required field Email: (text box)*required field Comment, Question, or Other Information: (paragraph box) Human Test: (text box - question to test if the submitter is human - "What is the month before July?" Answer "june" and "June") *required field Other requirements: Spam protection: adding a PHP routine which will check all the input boxes on the PHP form for http, CR and LF characters File must be saved allowing it to be called using <?php include "product_inquiry.php" ?> NOTE: please establish a Google Spreadsheet form on Google Documents that matches the above criteria to test your form.
