Create joomla form to collect data.

Create a form in joomla with 35 fields,
8 drop down fields, 1 file upload gif or jpeg 100 by 100 pixel limit, The rest of the fields are numeric, alpha and alphanumeric. Most fields are required some are not.
The confirmation
On the confirmation page the user will be shown all form entries made and given an option to edit entries or complete submission.
Thank you page.
All form submissions will be saved in the database and email sent to predefined email addresses
**Front end**
(1) The form
(2) publisher(joomla user Publisher) has access to query the database form submissions by date range. and ability to guery the database entries from up to five additional fields. The query results are to display on screen or download to excel spreadsheet
**Back end**
(1)database backup and restore button
(2)3 windows to add items to drop down menus.
(3)Edit Form Title up to 3 lines centered over form
(4)Edit Form footer up to 2 lines centered at the bottom of form
(5)Edit Thank you page
(6)Add/Edit Email recipients
## Deliverables
We need a joomla 1.0.X component or module or manbot that will install a a form to collect data.
We want web enabled cell phones to also be able to fill the form.(optional)
From Front end, any body can fill the form. specified users should be able to querry the data base by date range and or by values entered in to up to five fields. The results should be able to be downloaded to an excell spread sheet or print to screen.
From the back end we need to be able to
