Convert Table to CSS Div and Span

I have an old online survey that uses tables and no fieldsets / legends.? I have converted this into a new survey with less tables and fieldsets and legends.? I need someone who knows CSS to make the new survey look as much like the old as possible.

Winning bider will be given URL for each survey.? You will need to tell me what to do in the code to make the? new survey look as close to the old as possible.? Must work in FF 1.5+, IE 6+.
